Registration Checklist & Important documents
Thank you for choosing First Presbyterian Preschool for your family. The following checklist will help you through the registration process. If you have any questions, please do not hesitate to contact the preschool office at 419-756-0773.
Paperwork needed to attend preschool:
Registration Form: To be completed by parent or guardian and returned with a $45.00 registration fee (non-refundable).
Emergency Form: To be completed by parent or guardian and both sides to be filled out completely (all boxes and lines should filled out even immunization box).
Medical Form: To be completed and signed by Physician (form needs to have date of physical and physicians signature - physical is good for 1 year from the date put on medical form - needs to be an updated physical).
Photo Release Form/Policies and Procedures Form: To be completed by parent or guardian and returned.
Pick-up Authorization Form: To be completed by parent or guardian and returned. The people listed on this form will be able to pick-up your child/children. If they are not on this form, they will not be able to pick up your child/children.
Child Information Form: To be completed by parent of guardian and returned. This form goes to the teachers. Please make sure to put down all information needed for the teacher to get to know your child.
Supply Fees: There are due at the first week of preschool. Please do not pay until the first week of school.
